Similar to the Quick Menu's main Launch Pad tab which is already listed as a known issue. Unsure if related. Feel free to close if it is the same issue.

Closing and opening the World Actions bar realigns the icons, usually returning them to their intended position.

Closing and re-opening the Quick Menu returns the icons to being offset.
