Is this happening consistently? Or just this one instance?

This issue is affecting many users in the open beta, including myself. According to Ghost from the VRChat support team, the output log I sent has been forwarded to an engineer, and they are working on a potential fix.
Tony mentioned in the VRChat Discord that Fusl, who works on the API team at VRChat, requested that Tony create this Canny post and that this already has been fixed internally and awaiting review.
To answer your question, the issue is occurring consistently and affecting other users in the open beta. I suggest you consult your coworkers for more information.

RootVR: Any luck with this fix? Or is it still occurring?

It seems that the issue has been resolved. I haven't experienced it since the update, and I haven't seen any reports about it on Discord.
