Strangely, I had not chosen to leave the world. The game just left me in limbo, trapped under the world at 0,0,0. I was still receiving joins and leave notifications, but attempts to respawn were met with 'No Valid Spawn Points'
After about 5 minutes of poking at things my logfile bloated with an extra 20 megabytes of the following...

Unable to find a way out, I eventually left the world and rejoined.
