"Moderation Report Closed" notifications are duplicated

I have been suspicious and become fairly certain for a longer while now (almost) every "Moderation Report Closed" notification gets duplicated. For example, these two notifications created today at 13:40:56 UTC:

When viewing closed reports in the VRChat client, only 1 recently reported group had its report closed and actioned (the group was deleted or set private). Then this notification appeared twice, some hours later.
I think these duplicate notices also occur with avatars, emojis and stickers. I think I've observed this to have been occurring mostly since November 2025.

This may no longer be occurring. 4 unique notifications, 4 closed reports, 4 public avatars made unavailable to the public (1 or 2 authors).

For more data, I have received 118 or 155 "Moderation Report Closed" notifications today, and most of these notifications have been duplicated. Full logs are attached privately in ticket #656446 for diagnosing the issue.

Here's another duplicated notification from 15:24:40 UTC, which resulted in an avatar being made private:

Not all notifications are duplicated, such as this following closed report (timestamp redacted), but I notice no apparent action was taken on that user's profile:
