For instance, repro step is:

Run VRChat.exe in desktop mode
Press
Shift
key and continue to pressing
Type
Alt + Tab
and change active window to some other app
Stop pressing
Shift
key
Type
Alt + Tab
and activate VRChat window again
Type
ESC
key, then the safe mode is activated. Though the safe mode is bound to
Shift + ESC
and you don't press
Shift
key.

All key inputs seems behave as same. for example if you try

then the avatar continues to walk forward.

[1200] If you tab out of VRChat while pressing a button the game will continue to press it.

If you press a key like W and then tab out of VRChat window, the game thinks you are still pressing it.

Repro steps:

Hold down on W, or any other button for movement.
Press the Windows key, or use Alt + Tab to tab out of the VRChat window.

If you do this, you will notice that you are continuing to move forward. This is not the case in live currently.

This was driving me mad while I was recording clips with ShareX. 
My bind for screen capture is shift+prtsc, and once ShareX took focus away for a second, VRChat would think I continued holding shift which would throw me into Safe Mode when opening the menu.
I re-recorded my clips quite a few times before I figured out why it was throwing me into Safe Mode.
