This can occur in all avatars of the foot where the bone structure is close to straight.
It is mainly the left leg that bends.
The closer the leg bone is to a straight line, the more likely it is to bend. (However, if you try to avoid this and bend the knee too much, you will get an "not optimized for FBT" warning).

Blender's photo is to show that this model is a general FBT-oriented bone with a structure.

ni1chigo2115: why would you rig the foot like this? i see this as user error, not an IK issue.

BigSoulja: If this is indeed the case, both feet should point in opposite directions.
The fact that only one leg is facing the other way may be a problem with the "default IKPose animation retargeting" as discussed by Kung.
I'll post another picture, but this avatar's bone structure has the same problem, and I fixed it once in Blender to avoid the fatal part, but the legs are still unstable.
I get a warning when I tilt the UpperLeg and I don't know how to fix it.
