Hiyu
I was in an instance of Club Orion and talking to some people, when suddenly everyone went silent. Rejoining didn't fix it. I checked my audio settings in my Quick Menu and Voices had been changed to 0.

Thank you for submitting this bug. Do you have more information and details for what is happening. Any steps we can follow to reproduce the issue?
Unity Man
Scout - VRChat Head of Quality Assurance: All I have done was I installed the beta, joined a instance full of people who are in Open Beta and It just started placing people's voices in random places (Audio has flipped. Left is Right and Right is left.). That's all I did. World audio does not seem to be affected by this just only people's voices.

Scout - VRChat Head of Quality Assurance: Easy way to reproduce;
Go to the Suburban Lakehouse world
Hang around there with around 10+ people, get some avatars firing off audio effects to hasten it. Within about an hour (if not half that) the audio will break, guaranteed. And it'll tank the audio engine on EVERYONE in the instance over a span of minutes when it does.

Whaddageek: Thank you. I will give this a try.

Unity Man: Same exact issue, everything was fine and dandy, joined a world with 10+ people and instantly everyone's voice was mono, one directional, flipped, and loud as fuck.
Nothing fixed it sans reverting back to live.

Scout - VRChat Head of Quality Assurance: There's also an additional audio bug kicking around that will cause a player to become unable to speak. Changing microphone settings will not do anything, only a restart of VRChat will fix the problem.

Whaddageek: Thank you again. We are already tracking this related issue internally.

It absolutely does happen in live build, and has for months.
It's not even hard to cause; You run enough avatar audio sources long enough, the audio engine has a fit and can't keep up anymore.
